Looking for direction on a vendor or manufacturer that meet the requirements:

mATX or mITX

Width 13.5"
Depth 15"
Height 4"

Dual height PCI slots via PCI riser, will allow for the 4" height

www.silverstonetek.com - look at the ML08, FTZ01, RVZ01 (all basically the same chassis). Very close to your dimension limits.
Also look at the Zaber Sentry, NFC S4 Mini in the custom projects forum.
